30-day survey-prep pilot evaluation checklist

Agree the scope and pass criteria before starting. Record observed results, not expected savings. Use record references instead of resident or employee details. This is a purchasing worksheet, not a compliance assessment.

1. Define the baseline

Choose three recurring tasks. Record minutes spent, missing evidence, duplicate entry and the person doing the work in your current process.

2. Confirm coverage and access

Confirm your state, license type, roles, data handling and required connections. Name what is included, excluded or dependent on manual work.

3. Check a real workflow

Locate a survey finding, its draft and due date. Review a credential and a documentation record. Have the administrator and DON repeat the tasks.

4. Verify review and follow-up

Check source accuracy, update frequency and who corrects an error. Confirm how you handle review, editing, submission and recurring follow-up outside any unsupported workflow.

5. Check portability and cost

Confirm which records you can retain, the method of retrieving them, any limits, the monthly cost and cancellation terms. Do not assume a particular export is available.

6. Make the day-30 decision

Compare the same tasks with your baseline. Record measured time, unresolved gaps and whether each pass criterion was met. Choose continue, extend by agreement or stop; name who approves the decision.
